Job summary

Enlyte is seeking a Field Case Manager in Greenville, SC. This full-time, hybrid role requires regular local travel to attend in-person patient appointments.

You will coordinate with physicians, employers, and patients to implement treatment plans that restore work capability while ensuring cost-effective healthcare services. The position emphasizes clinical expertise, independent case management, and collaboration with a multidisciplinary team.

Qualifications

• Associates or Bachelor’s Degree in Nursing required or related field.
• Active Registered Nurse (RN) license in good standing required.
• URAC-recognized certification in case management (CCM, CDMS, CRC, CRRN or COHN, COHN-S, RN-BC, ACM, CMAC, CMC) preferred or required.

Responsibilities

• Demonstrate knowledge and competency in applying case management standards of practice.
• Advise on treatment plans using knowledge of injuries, medications, comorbidities, and job duties.
• Interview injured workers to assess recovery and determine occupational vs. non-occupational status.
• Collaborate with physicians/providers to tailor treatment plans.
• Modify job duties with employers to facilitate early return to work.
• Evaluate and adjust case goals based on progress and effectiveness.
• Independently manage workload and prioritize cases.
• Attend appointments when needed and prepare status updates for clients.
