Job summary

Commonwealth Care Alliance is seeking a Registered Nurse Case Manager to support dually-eligible enrollees in Massachusetts. This mobile, field-based role requires travel to member homes and community settings to coordinate care and resources.

You will lead the care team, perform assessments, and help beneficiaries navigate Medicare/Medicaid benefits while promoting self-management and preventive care.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited nursing program required.
• A Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs. required.
• Prior experience in care coordination, case management, or working with dual-eligible populations preferred.
• Medicaid and/or Medicare managed care experience preferred.
• Clinical Field/ Community Based Training a Plus.

Responsibilities

• Engage with enrollees in their homes and communities to build a complex care management relationship.
• Coordinate with healthcare providers and community resources to ensure seamless care transitions.
• Perform Comprehensive Assessments and functional assessments such as MDS-HC or successor.
• Develop and implement care plans with regular updates as needs change.
• Lead interdisciplinary care team to address medical and non-medical needs.
• Oversee utilisation of long-term services to help enrollees stay in their desired location.
• Assist members in accessing housing, transportation, food assistance, and social services.
• Educate members about benefits under Medicare and Medicaid and about managing chronic conditions.
