Job summary

Commonwealth Care Alliance Inc. is seeking a Mobile Integrated RN Care Manager in Massachusetts to provide monitoring, follow-up and clinical care management to dually eligible enrollees with complex medical, behavioral, and social care needs.

This role focuses on coordinating health services and community resources to improve outcomes and quality of life. The position requires travel to enrollees' homes and other community settings, collaboration with an interdisciplinary team, and adherence to

Qualifications

• Associates of Science (A.S) degree in nursing required.
• Registered Nurse with ability to independently serve people with complex needs required.
• Case Management Certification preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build a complex care management relationship.
• Coordinate with healthcare providers and community resources to ensure seamless care transitions.
• Perform required assessments including Comprehensive Assessment and functional assessments.
• Develop and implement care plans with ICT, updating as status changes.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
