Job summary

Commonwealth Care Alliance is seeking a Hybrid Registered Nurse Case Manager to coordinate care for dually eligible enrollees with complex medical, behavioral, and social needs in Massachusetts. This mobile role requires in-home and community visits to assess, plan, and implement care with an interdisciplinary team.

The RN must hold a Massachusetts license and have experience in care coordination or case management, with willingness to travel within the territory. A sign-on bonus is offered.

Qualifications

• Associate of Science in nursing from an accredited program.
• Registered Nurse licensed in Massachusetts.
• Medicaid/Medicare managed care experience preferred.
• Care coordination/case management experience preferred.
• Clinical field/community-based training is a plus.

Responsibilities

• Engage with enrollees in homes and community settings to establish a complex care management relationship.
• Lead the interdisciplinary care team to create holistic care plans.
• Coordinate with providers and community resources for seamless care transitions.
• Educate members about Medicare and Medicaid benefits and services.
• Conduct assessments (Comprehensive, MDS-HC, risk assessments) and monitor status.
• Travel to member, provider and community visits as required.
