Job summary

Commonwealth Care Alliance Inc. is seeking a Mobile Registered Nurse Case Manager to coordinate care for dually-eligible enrollees with complex medical, behavioral, and social needs in Massachusetts.

In this role you will lead a multidisciplinary team, conduct comprehensive assessments, develop care plans, and connect members with housing, transportation, and other community resources to improve health outcomes and quality of life.

Qualifications

• Associates of Science in nursing required.
• Registered Nurse license required.
• Care coordination/case management experience preferred.
• Medicaid/Medicare experience preferred.
• Clinical field/community-based training a plus.

Responsibilities

• Engage with enrollees in homes and communities to build care management relationships.
• Liaise between providers, resources, and enrollees to ensure seamless care.
• Conduct required assessments (Comprehensive Assessment, MDS-HC, Risk Assessments).
• Develop and update care plans with ICT and interdisciplinary teams.
• Coordinate with community organizations to ensure resource access.
• Educate members on benefits, services, and preventive care.
• Assist with transitions and preventive health strategies.
• Travel to member/provider/community locations as required.

Education

Associate of Science (A.S.) in Nursing

Registered Nurse license

Case Management Certification (preferred)
