Job summary

Commonwealth Care Alliance Inc. is seeking an Integrated Care RN Case Manager for Massachusetts. You will visit enrollees at home and in community settings, coordinating care with providers and resources to improve outcomes and quality of life.

You will lead an interdisciplinary team, conduct comprehensive assessments, and develop care plans while educating members about Medicare/Medicaid benefits and preventive care. Travel across MA is required.

Qualifications

• Associates of Science degree in nursing required.
• Active RN license required.
• Experience in care coordination or case management preferred.
• Medicare/Medicaid managed care experience preferred.
• Clinical field/community-based training a plus.

Responsibilities

• Engage enrollees in home and community settings to build complex care management relationships.
• Coordinate care between healthcare providers, resources, and enrollees.
• Conduct comprehensive assessments including MDS-HC as applicable.
• Develop and implement care plans with updates as statuses change.
• Lead the interdisciplinary care team to create holistic care plans.
• Oversee enrollment in long-term services to keep enrollees in their location of choice.
• Educate members about benefits under Medicare and Medicaid.
• Follow up after hospitalizations to ensure continuity of care and prevent readmissions.
• Assist members in accessing community resources such as housing and transportation.
