Job summary

Commonwealth Care Alliance seeks a Mobile RN Case Manager to provide monitoring, follow-up, and clinical care management for dually-eligible enrollees with complex needs.

You will coordinate health services with community resources, lead the ICT, ensure care plan implementation, and support transitions with primary care teams. This is a mobile, full-time Massachusetts-based role with travel to member homes and clinics.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited nursing program.
• Current unrestricted RN license in Massachusetts.
• Case Management Certification is highly preferred.
• Medicaid and/or Medicare managed care experience preferred.
• Prior experience in care coordination or case management for dual-eligible populations preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build an effective, complex care management relationship.
• Coordinate with healthcare providers and community resources to ensure seamless communication and care transitions.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Monitor enrollee utilization of long-term services and supports to enable staying in their chosen location.
• Educate members about benefits and services under Medicare and Medicaid; promote self-management.
