Job summary

Commonwealth Care Alliance is seeking a Hybrid RN Care Manager to coordinate complex care for dually-eligible enrollees across Western Massachusetts. The role emphasizes in-home assessments, care plan development, and collaboration with an interdisciplinary team to improve outcomes.

The position requires an ASN in nursing, an active RN license, and eligibility for Case Management Certification. Travel within MA is required; vaccination and driver’s license are prerequisites.

Qualifications

• Associates of Science (A.S) degree in nursing from an accredited nursing program required.
• A Registered Nurse with the ability to independently serve people with complex medical, behavioral, and social needs required.
• Case Management Certification is highly preferred.
• Must have valid driver’s license, vehicle and verifiable insurance.
• Influenza vaccination is a requirement of this position.

Responsibilities

• Engage with enrollees in their homes and community settings to establish complex care management relationships.
• Liaise between healthcare providers, resources, and enrollees for seamless communication and care transitions.
• Perform Comprehensive Assessments and MDS-HC Functional Assessments as required.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and non-medical needs.
• Coordinate with community-based organizations to avoid duplication of services and ensure access to resources.
