Job summary

Commonwealth Care Alliance is seeking a mobile RN Case Manager to deliver integrated care for dually-eligible enrollees in Massachusetts. The role emphasizes in-home assessments, care planning, and coordination with ICT and community resources to improve health outcomes and quality of life.

You will travel to enrollees' homes and community sites, collaborate with providers, assess needs, support transitions, and promote preventive care while navigating Medicare/Medicaid benefits.

Qualifications

• RN with current MA license required.
• Experience in care coordination or case management preferred.
• Medicaid/Medicare managed care experience preferred.

Responsibilities

• Engage with enrollees in homes and community settings to build complex care management relationships.
• Lead the interdisciplinary care team to develop holistic care plans.
• Coordinate with providers and community resources to ensure smooth care transitions.
• Educate members about benefits, services, and preventive care.
• Follow up after hospitalizations to ensure continuity of care.
