You will coordinate care for high-risk patients with complex medical and social needs, provide chronic disease education, perform medication reconciliation, and help patients navigate the healthcare system.

Qualifications

• BSN required.
• Minimum 2 years of clinical nursing experience; 3 years in case/care management preferred.
• Current MD RN license (or multi-state/compact).
• Managed Care experience preferred.
• Community-based acute care setting experience preferred.
• EPIC/Compass Rose experience preferred.

Responsibilities

• Coordinate care for high-risk patients across providers and settings.
• Provide chronic disease education and medication reconciliation.
• Help patients navigate the healthcare system.
• Support care coordination to reduce avoidable hospital and ED utilization.
• Contribute to programs improving health outcomes.
