Johns Hopkins HealthCare LLC in White Marsh, MD is seeking a Case Manager Registered Nurse to join a newly established program within the Johns Hopkins Community Physicians team. You will coordinate care for high-risk patients with complex medical and social needs, provide chronic disease education, perform medication reconciliation, and help patients navigate the healthcare system.

Qualifications

• BSN required.
• Minimum 2 years of clinical nursing experience; 3 years in case/care management preferred.
• Current MD RN license (or multi-state/compact).
• Managed Care experience preferred.
• Community-based acute care setting experience preferred.
• EPIC/Compass Rose experience preferred.

Weekly Working Hours: Monday-Friday 8:00am-5:00pm ( Hybrid position- 4 days onsite | 1 day remote, after 90 days of employment)

Join our team and be part of a newly established case management program dedicated to serving our community by delivering high-quality, patient-centered care. The Case Manager Registered Nurse is an integral member of the primary care team, working collaboratively with providers to engage patients in improving chronic disease self-management, coordinating care, and reducing unnecessary healthcare utilization.

In this role, you will support high-risk patients with complex medical and social needs by coordinating care across multiple providers and settings, providing chronic disease education, performing medication reconciliation and education, and helping patients navigate the healthcare system. You will play a key role in implementing programs designed to improve health outcomes and reduce avoidable hospital and emergency department use.

Qualifications:

• Bachelor's Degree BSN (Required)

Salary Range:

Minimum 43.61/hour - Maximum 67.59/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
