Job summary

CareSource/Commonwealth Care Alliance (CCA) is seeking a Mobile Integrated Care RN Care Manager in Massachusetts. The role focuses on monitoring, follow-up, and clinical management for dually-eligible enrollees with complex needs in community settings.

The RN will lead the interdisciplinary care team, coordinate with providers and community resources, and support enrollee access to housing, transportation, and services while promoting preventive care and self-management.

Qualifications

• ASN degree in nursing from an accredited program.
• Active MA RN license is required.
• Experience in care coordination or case management preferred.
• Medicare/Medicaid managed care experience preferred.
• Clinical field/community based training is a plus.

Responsibilities

• Engage enrollees in their homes and community settings to establish complex care management relationships.
• Coordinate with healthcare providers, resources, and enrollees for seamless care transitions.
• Conduct comprehensive assessments including MDS-HC or successor.
• Lead the interdisciplinary care team to create holistic care plans addressing medical and social needs.
• Oversee enrollee utilization of long-term services and supports and ensure access to housing, transportation, and services.
• Educate members about benefits, preventive care, and disease management; promote self-management.
