MICare Management in Canton, MI is seeking a compassionate Registered Nurse (RN) Case Manager to join our growing team. This full-time hybrid role combines in-office collaboration with remote work to coordinate care for patients with chronic and complex conditions.

You will guide patients through the healthcare system, develop individualized care plans, and partner with providers and families to improve outcomes.

Qualifications

• Active and unrestricted RN license in Michigan.
• 1–2 years of clinical experience in case management or related settings.
• Strong care coordination, discharge planning, and chronic disease management knowledge.
• Familiarity with healthcare regulations and documentation standards.
• Experience with EMR/EHR systems.

Responsibilities
