Job summary

Responsibilities

• Conducts comprehensive assessment of biomedical, psychological, social, and functional needs to gauge impact on recovery.
• Develops personalized patient-centered care plans to optimize the patient’s care experience.
• Engages patients and families as part of the care team through advocacy and ongoing communication.
• Coordinates care and advocates for options and services across care settings.
• Advocates for appropriate delivery of services within the patient’s health plan and collaborates with care team.
• Supports transitions between care settings and post-acute care providers.
• Participates in process-improvement activities to reduce risk and improve outcomes.
