MICare Management in Canton, MI is seeking a Spanish-speaking Registered Nurse (RN) Case Manager for a hybrid role. You will guide patients through the healthcare system, coordinate care, and support families to understand treatment plans.

The ideal candidate has an active MI RN license, fluent Spanish, 1–2 years clinical experience, and knowledge of care coordination and discharge planning. Remote-work options are available as part of the flexible schedule.

Qualifications

• Active and unrestricted RN license in Michigan.
• Fluent in Spanish.
• Minimum of 1–2 years of clinical experience.
• Case management, home health, rehab, or acute care experience preferred.
• Strong understanding of care coordination, discharge planning, and acute/chronic disease management.
• Knowledge of healthcare regulations, documentation standards, and compliance requirements.
• Experience with EMR/EHR systems.
• Excellent clinical judgment and critical thinking skills.
• Strong communication and patient education abilities.
• Ability to work independently, prioritize tasks, and manage a flexible schedule.
• Valid driver’s license and reliable transportation.

Responsibilities

• Communicate with patients and families in Spanish throughout the case management process.
• Navigate and coordinate care across healthcare systems.
• Identify and address potential medical concerns.
• Develop and implement individualized care plans.
• Coordinate appointments, referrals, follow-up care, and services.
• Support patients with chronic and complex conditions.
• Provide patient education and help patients understand their treatment plans.
• Collaborate with providers, families, and interdisciplinary care teams.
• Maintain accurate documentation and ensure compliance with healthcare regulations.
• Manage an assigned caseload while staying organized and responsive to patient needs.
