Mass General Brigham Community Physicians, Inc. is seeking a Registered Nurse to coordinate care, support enrollment, and plan community services. You will work with patients, families, and providers to develop comprehensive care plans and ensure seamless transitions.

The role requires an MA RN license, 2+ years in case management or discharge planning, and strong communication skills. Hybrid schedule in Jamaica Plain and fieldwork with remote work options are provided.

Competenze

• RN license in MA required.
• 2+ years of case management, utilization review, or discharge planning experience.
• Ability to coordinate care with patients, families, and providers.
• Knowledge of managed care is a plus.

Mansioni

• Coordinate care within the facility and arrange referrals as needed.
• Support outreach and enrollment services to meet program eligibility.
• Develop and coordinate community service plans with providers and families.
