Centene is seeking a Registered Nurse Care Manager for Duals AIP adult members in a hybrid role. You will perform home visits, assess needs, develop personalized care plans, and coordinate with providers to ensure high-quality, cost-effective care.

Schedule is Monday–Friday, 8:00 AM–5:00 PM CST with no on-call duties. Mileage for member visits is reimbursed, and you will work from a flexible home office in the South Dallas area.

Competenze

• Graduate from an accredited School of Nursing with current RN licensure.
• 4–6 years of related clinical nursing experience; BSN preferred.
• Experience in care coordination or care management is desirable.

Mansioni

• Assess complex/high acuity members and coordinate plans for outcomes.
• Develop and monitor long-term care plans and coordinate providers and resources.
• Coordinate between members/families and care teams to ensure appropriate services.
• Document care management activities and maintain compliance with guidelines.
• Educate members and caregivers on services, options, and benefits.
• Collaborate with leaders to improve quality and efficiency of care.
