Job summary

Centene Corporation in Brownsville, TX is seeking a registered nurse care coordinator to assess and coordinate all aspects of medical and supporting services for complex and high-acuity populations. You will develop personalized care plans and educate members and families to receive high-quality, cost-effective care.

This hybrid role includes home visits and remote work, with a Mon-Fri 8:00 AM–5:00 PM CST schedule. Travel to service areas in Brownsville, Harlingen, and San Benito is required.

Qualifications

• Graduate from an Accredited School of Nursing or a Bachelor's degree with 4–6 years of related experience.
• RN license active in the state or NP with current licensure; RUG certification within 90 days of hire.

Responsibilities

• Evaluates the service needs of the most complex or high risk/high acuity members and recommends a plan for the best outcome.
• Develops and continuously assesses long-term care plans and collaborates with the care management team to identify providers, specialists, and community resources.
• Coordinates between member/family/caregivers and care provider team to ensure appropriate person-centered care.
• Monitors care plans and member status; revises plans to meet goals and needs.
• Collects, documents, and maintains all member information to ensure compliance with guidelines.
• Educates members and families on disease processes, care gaps, and benefits.
• Travel is required for member visits.
