Neighborhood Healthcare in Temecula, CA is seeking a Registered Nurse Care Manager for Enhanced Care Management to support ECM patients with complex medical needs. This hybrid position requires on-site participation and occasional site travel for meetings and trainings.

You will coordinate care, perform medication reconciliation, schedule referrals, and monitor outcomes in the care management platform while promoting health literacy for patients and families.

Formación

• Associate's degree in nursing required; CA RN license required.
• BLS certification required by hire; online courses not accepted.
• 2 years clinical, outpatient, or general medicine experience preferred; 3 years nursing/case management required.
• Experience with community outreach and behavioral health preferred.

Responsabilidades

• Manages a caseload of patients with complex medical needs.
• Engages and activates patients toward health goals.
• Coordinates care management and referrals; tracks outcomes in ECM platform.
• Performs medication reconciliations with input from providers.
• Facilitates transitions of care with hospitals.
