Job summary

Amity Foundation in Moreno Valley, CA seeks a Nurse Care Manager for our Enhanced Care Management (ECM) – Community Supports program. You will manage a caseload of Members with complex medical needs, coordinate care, perform medication reconciliations, and educate Members and families to improve health outcomes.

The role emphasizes collaborative teamwork, evidence-based communication, and seamless transitions of care with hospitals and IEHP, with travel within the service area as needed.

Qualifications

• Registered Nurse (RN) or Licensed Vocational Nurse (LVN) licensed in California.
• Minimum 5 years nursing experience with 2+ years in care management or population health.
• Strong analytical skills and ability to translate data into actions.

Responsibilities

• Manage caseload of ECM Members with complex medical needs.
• Perform medication reconciliations with involvement of PCP and pharmacy.
• Educate Members and families on chronic conditions and care plans.
• Coordinate with ECM Care Team for transitions of care.
• Engage Members to meet health goals and document outcomes.
• Track assessments, screenings, and health need plans.
• Travel locally to support program initiatives and trainings.
