Job summary

Neighborhood Healthcare is seeking a Care Coordinator for Enhanced Care Management in Temecula, CA. Qualifications

• High school diploma or equivalent is required.
• Two years of healthcare, behavioral health, or social services experience is required.
• Active CA driver’s license and proof of car insurance required.
• Medical Assistant or Phlebotomy certificate preferred.

Responsibilities

• Coordinates care for complex patients and navigates social services.
• Schedules appointments and manages referrals.
• Travels to other Neighborhood sites as needed.
• Documents in ECW in a timely manner; supports quality improvement.
• Participates in team case reviews and coordinates with ECM RN(s).
• Assists with transportation and durable medical equipment arrangements.
