Neighborhood Healthcare in Temecula, CA seeks a Care Coordinator for Enhanced Care Management to coordinate care for complex patients and connect them with community resources addressing social determinants of health (SDOH). The role includes appointment scheduling, referral management, and travel to other Neighborhood sites as needed.

The hybrid position requires a high school diploma and about two years in healthcare or social services, plus an active CA driver’s license.

Formación

• High school diploma or equivalent required.
• Two years of healthcare, behavioral health, or social services experience required.
• Active CA driver’s license and proof of car insurance required.
• Medical Assistant or Phlebotomy certificate preferred.

Responsabilidades

• Provides ongoing monitoring of the Targeted Engagement List (TEL).
• Assigns patients to ECM team members based on risk and data.
• Works with a caseload of low-risk patients.
• Conducts periodic telephonic outreach and follow-up per Shared Care Plan.
• Supports ECM team members with delegated tasks.
• Coordinates appointments and referrals for physical/behavioral health.
• Collaborates with ECM team on care issues; participates in reviews.
• Connects ECM patients to social services and supports.
• Uses motivational interviewing to engage patients.
• Assists with transportation and durable medical equipment arrangements.
• Coordinates with patients in accessible settings per IEHP guidelines.
• Contributes to quality improvement activities.
• Documents care coordination in ECW timely.
