Job summary

Heritage Health Network (HHN) seeks an experienced Registered Nurse Care Manager (RNCM) to provide clinical oversight for our Enhanced Care Management (ECM) program. The RNCM will review charts, assess plans, identify gaps, and guide Lead Care Managers, with a mix of remote clinical work and field-based activities.

The role emphasizes collaboration with a multidisciplinary team and in-person member engagement as required.

Qualifications

• Active California RN license required.
• Minimum 2 years nursing experience.
• At least 1 year in care management or leadership.
• Experience with vulnerable populations and complex needs.

Responsibilities

• Oversee care plans and assess member charts in EMR/EHR.
• Provide clinical guidance to Lead Care Managers on complex cases.
• Engage in field visits and in-person member engagement as needed.
