Job summary

UVA Health in Charlottesville, VA is seeking a Registered Nurse Care Coordinator for the Digestive Health Clinic to connect patients, providers, and services. This role blends direct patient care with care coordination and a stable hybrid work structure.

You will guide patients with Crohn’s, ulcerative colitis, celiac disease, and other GI conditions through testing, treatment, and long-term management, leveraging strong nursing judgment and collaborative teamwork.

Qualifications

• Education: BSN from an accredited program.
• RN licensure in Virginia with AHA BLS certification.
• 1 year RN experience required; 3 years preferred; specialty board certification preferred.

Responsibilities

• Coordinate diagnostic testing, procedures, and follow-up.
• Manage medication questions, refills, and prior authorization support.
• Respond to patient concerns by phone and via MyChart with nursing judgment.
• Educate patients on care plans, testing, medications, and next steps.
• Coordinate care across GI providers and multidisciplinary teams.
