Job summary

Qualifications

• High school diploma or equivalent is required.
• Medical Assistant certification, LPN license, or related care coordination training is preferred.
• BLS certification is required.
• Clinical experience in a medical office or outpatient care setting is required.
• Knowledge of population health and care coordination workflows is preferred.
• Familiarity with Epic EMR or other EMR systems is a plus.

Responsibilities

• Care Coordination & Planning: Collaborates with providers, patients, families, and teams to create, implement, and monitor care plans.
• Patient Communication & Support: Serves as a point of contact for patients and families regarding treatment plans and services.
• Authorization & Resource Coordination: Obtains prior authorizations for medications, procedures, and equipment.
• Documentation & Communication: Documents interactions and updates in the patient chart and communicates with care team.
• Clinical Support (As Needed): Assists with back-office duties during staffing shortages and routine tasks.
• Team Collaboration & Quality Improvement: Participates in huddles and quality initiatives to support clinic goals.
• Other Duties: Performs clerical tasks and may float across departments as needed.
