As a Registered Nurse Care Coordinator within UVA Health’s Digestive Health Clinic, you will serve as the clinical connection point between patients, providers, and essential services . This opportunity combines meaningful patient relationships, independent nursing practice, specialty development, and a predictable hybrid work structure.

The Role

You will help patients with Crohn’s disease, ulcerative colitis, celiac disease, chronic abdominal pain, and other complex gastrointestinal conditions to navigate diagnostic evaluation, treatment, and long-term disease management. Rather than performing isolated tasks, you will assess evolving needs, investigate clinical concerns, and help translate provider care plans into coordinated action.

Your average day may include:

• Coordinating diagnostic testing, procedures, laboratory monitoring, and follow-up
• Managing medication questions, refill requests, and prior authorization support
• Responding to patient concerns by phone and MyChart using sound nursing judgment and critical thinking
• Educating patients about care plans, testing, medications, and next steps
• Coordinating care across GI providers, specialty services, and multidisciplinary teams
• Identifying clinical issues, gathering relevant information, and independently advancing appropriate solutions
• Building trusted relationships with patients who require ongoing monitoring and support
• Partnering with physicians, nurses, and clinic staff in a collaborative, team-oriented practice

The position follows a Monday through Friday, standard business hours schedule with no nights, weekends, or major holidays . A consistent hybrid model combines onsite clinical collaboration with remote work aligned to provider schedules. Schedules are generally established in advance, with flexibility expected to support clinic coverage when team needs change.

GI experience is valuable, but nurses from medical-surgical, ICU, acute care, ambulatory specialty, infusion, chronic disease management, and existing care coordination backgrounds may also bring highly transferable skills. Strong recent clinical experience, organization, communication, and independent problem-solving skills are essential.

MINIMUM REQUIREMENTS:

Education : Bachelor of Science in Nursing from an accredited nursing program.

Experience : 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.

Specialty Board Certification strongly preferred

License/Certification : Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification is required.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, and bending/stooping. Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids, and infectious diseases.

The pay range for this role is $88,212.80 - $115,793.60 annually. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
