Job summary

Qualifications

• Bachelor of Science in Nursing required.
• 1 year of registered nursing experience required, 3 years of related and relevant registered nursing experience strongly preferred.
• Specialty Board Certification strongly preferred.

Responsibilities

• Serve as a trusted clinical point of contact for a provider-specific patient population.
• Respond to patient messages and clinical questions with clarity, compassion, and sound nursing judgment.
• Coordinate follow-up appointments, laboratory testing, radiology studies, procedure dates, and individualized care plans.
• Educate patients and families about procedures, preparation requirements, treatment expectations, and next steps.
• Monitor patient progress and help ensure timely follow-through across the care journey.
• Support provider clinic activities and communicate scheduling needs to Access staff.
• Observe procedures when needed to strengthen understanding of treatment plans and post-procedure requirements.
• Provide cross-coverage for fellow care coordinators during absences and planned time away.
