You will coordinate comprehensive care for individuals with developmental disabilities in a residential setting, liaising with families, healthcare providers, and community resources.

The Nurse Care Coordinator will lead health assessments, document nursing notes in the EMR, manage hospitalizations and follow-ups, and train direct care staff to ensure

Qualifications

• Valid CT RN license required.
• DDS nursing orientation upon hire or 6 months after hire.
• Certified Health care provider CPR (BLS) required.
• 3 years of nursing experience with people with developmental disabilities.
• Basic computer knowledge (Outlook, MS Office) required.
• Valid driver’s license.
• Clear criminal record and background checks as per state requirements.

Responsibilities

• Coordinate and manage treatment, care, appointments, and follow-up for assigned caseload.
• Complete required Health Assessments on supported individuals.
• Document FOCUS nursing notes in the EMR system.
• Manage hospitalizations and ensure timely care.
• Conduct biweekly home visits to supported individuals.
• Provide education and training to direct care providers to uphold continuity of care.
• Act as liaison between individuals, families, providers, and community resources.
