Job summary

Wisconsin Nurses is seeking a Registered Nurse (RN) Care Coordinator to supervise personal care workers and deliver services to consumers. This full-time, salaried position involves home visits, consumer assessments, and developing plans of care in collaboration with the care team.

The role requires coordinating physician orders, conducting 60-day plan reviews, ensuring accurate documentation, and training staff.

Qualifications

• Must hold current RN licensure in Wisconsin.
• At least one year of supervisory or administrative experience.
• Experience in personal care, home health, hospice, or rehabilitation preferred.
• Strong written and verbal communication skills.
• Knowledge of Wisconsin Medicaid system is preferred.

Responsibilities

• Review plans of care and ensure RN oversight of personal care workers.
• Visit consumers' homes and assess environment and needs.
• Obtain physician orders and renew them as required.
• Maintain accurate medical and service records and timely documentation.
• Provide training and supervision to personal care workers.
