• Valid CT RN license required.
• DDS nursing orientation upon hire or 6 months after hire.

• 3 years of nursing experience with people with developmental disabilities.
• Basic computer knowledge (Outlook, MS Office) required.
• Valid driver’s license.
• Clear criminal record and background checks as per state requirements.

Responsibilities

• Coordinate and manage treatment, care, appointments, and follow-up for assigned caseload.
• Complete required Health Assessments on supported individuals.
• Document FOCUS nursing notes in the EMR system.
• Manage hospitalizations and ensure timely care.
• Conduct biweekly home visits to supported individuals.
• Provide education and training to direct care providers to uphold continuity of care.
• Act as liaison between individuals, families, providers, and community resources.

**Nurse Care Coordinator**Stratford, CT Full-time, Monday through Friday*Hybrid role combining in-field, in-office, and remote work**Professional Nursing Position (RN)*KenCrest is seeking a **Nurse Care Coordinator** (RN) to provide comprehensive care coordination and management services to an assigned caseload of individuals with intellectual and developmental disabilities in a residential setting. This role focuses on optimizing health outcomes, improving quality of life, and reducing healthcare costs for supported individuals by ensuring those individuals receive appropriate, timely, and efficient care.
