Job summary

PrideGlobal is seeking an experienced Registered Nurse (RN) Care Manager for a hybrid position combining field visits with remote care coordination to support older adults and individuals with complex health needs.

The role involves in-home assessments, care-plan development, and cross-setting coordination, with roughly six visits weekly in the Bronx territory and a flexible Long Island work setup.

Qualifications

• Active NY RN license.
• Associate degree in nursing required; BSN preferred.
• Minimum 3 years of RN experience.
• Experience in MLTC, home care, geriatrics or case management preferred.
• Strong communication, organizational, assessment and critical-thinking skills.
• Ability to work independently in a hybrid environment.
• Bilingual skills are a plus.
• Field Schedule: ~6 home visits per week.
• Training: Onsite at Queens office.
• Salary: $100,000–$130,000 per year.

Responsibilities

• Conduct in-home assessments to evaluate members’ clinical, functional, and social needs.
• Develop and manage individualized care plans in collaboration with physicians and interdisciplinary teams.
• Coordinate care across home care, hospitals, rehabilitation facilities, and long-term care settings.
• Complete approximately 6 home visits per week within the Bronx territory.
• Monitor member progress and adjust care plans to support optimal outcomes.
• Support safe discharge planning and help reduce hospital readmissions.
• Communicate effectively with members, families, caregivers, and healthcare providers.
• Maintain accurate and timely clinical documentation in accordance with applicable guidelines.
• Advocate for members and connect them with appropriate healthcare and community resources.
