Job summary

St. Luke’s Health Partners is seeking an experienced RN Care Advisor to ensure coordinated, high-quality care across the patient journey. You'll conduct assessments, develop patient-centered goals, and educate and advocate for patients and families.

Join a multidisciplinary team and coordinate with physicians and care teams. The role is hybrid (50% on-site / 50% remote) in the Boise area, with a focus on improving outcomes and patient experience.

Qualifications

• Bachelor's of Nursing Degree (BSN) required.
• Minimum of 2 years RN experience.
• RN utilization management or catastrophic care management experience preferred.
• Current RN License in the State of Practice

Responsibilities

• Assess patients' health needs and develop individualized care plans.
• Coordinate with physicians and care team to implement care plans.
• Educate patients and families and provide self-management support.
• Advocate for patients and navigate care across settings.
• Measure and improve quality outcomes for patients and populations.
• Perform ongoing education for patient and family on diagnosis and treatments.
• Handle additional duties as assigned
