Job summary

St. Luke’s Health System is seeking a Registered Nurse (RN) to serve as a clinical resource supporting outreach activities with triage, assessment, and escalation support for Outreach Coordinators.

The role emphasizes coordination with providers, care teams, and community resources to improve patient outcomes and population health. The position is full-time, onsite at the Boise location, with a schedule of 8am–5pm, Monday–Friday, and requires a BSN, RN license, and experience in ambulatory care

Qualifications

• BSN required and active RN license.
• 2–3 years in ambulatory care, care management, triage, or population health preferred.
• Strong clinical assessment, critical thinking, and communication skills.
• Ability to work with multidisciplinary teams and manage competing priorities in a fast-paced environment.
• Active RN license and BLS certification.

Responsibilities

• Conduct telephonic and electronic triage of care management referrals.
• Serve as escalation pathway for Outreach Coordinators when clinical judgment is required.
• Assess patient risk factors and determine levels of care and follow-up.
• Coordinate with providers and interdisciplinary teams to address urgent patient needs.
• Provide patient education related to symptoms, treatment plans, and self-management.
• Document assessments, interventions, and care coordination in the EMR.
• Support quality, care management, and population health initiatives through proactive patient engagement.
