Job summary

St. Luke’s Health Partners in Idaho is seeking an experienced RN Care Advisor to coordinate high-quality patient care across the healthcare journey.

You will conduct assessments, develop patient-centered goals and care plans, and educate patients and families to support self-management. In this role you join an interdisciplinary team and enjoy a Monday–Friday schedule with a focus on independent patient management after training.

Qualifications

• Bachelor's of Nursing Degree (BSN) is required.
• Current RN License in the State of Practice.
• 2 years RN experience preferred; utilization management or catastrophic care management experience preferred.

Responsibilities

• Coordinate and manage patient care plans with patients, families and physicians.
• Assess patient knowledge and provide education and self-management support.
• Develop patient-centered goals and care plans throughout the healthcare journey.
• Advocate for patients and coordinate care with the care team.
• Perform quality audits and chart reviews as part of daily duties.
