Job summary

Saint Luke’s East Hospital in Lees Summit, MO is seeking a Regional Nurse Care Coordinator for the Population Health team. This RN role focuses on coordinating care for a diverse patient population, largely over the phone, with no nights or weekends and a 50% hybrid option in KC region.

The role requires an RN license (MO/KS) and 3–5 years of experience in primary care or transitions of care, with a Bachelor's degree.

Responsibilities

• Engage with regional Saint Luke’s Physician Group clinics to improve patient care within specified payment structures.
• Drive engagement strategies to improve physician and staff patient care practices using clinical guidelines and benchmarks.
• Collaborate with patients, physicians, practice teams, and health plans to guide and coordinate care and support the medical home.
