Job summary

Humana Healthy Horizons in Indiana seeks a Care Coordinator 2 (Field Care Manager 2) to assess member needs and guide families toward appropriate resources. This field role supports Indiana PathWays for Aging, with hybrid home/onsite work, 40 hours weekly, and travel required to meet with members.

The ideal candidate is an Indiana-licensed RN with 2+ years of clinical case management experience, strong IT skills (Word/Excel/Outlook), and excellent communication. Residence in Indiana is required.

Qualifications

• Licensed Registered Nurse (RN) in Indiana without restrictions.
• At least two years of clinical nursing experience in case management or similar health care services.
• Proficient with Microsoft Word, Excel, and Outlook.
• Excellent communication and interpersonal skills for internal and external stakeholders.
• Strong critical thinking, organization, and problem-solving abilities.
• Ability to manage multiple priorities in a fast-paced environment.
• Experience using electronic medical records (EMR).
• Must reside in Indiana.

Responsibilities

• Coordinate medical and non-medical services for members.
• Develop longitudinal, trusting relationships with members to improve care quality and continuity.
• Collaborate with Service/Transition coordinators and care team to address needs.
• Coordinate care and benefits with Medicare payers and providers as appropriate.
• Serve as primary ICT contact to ensure member needs are met.
