Job summary

Qualifications

• Licensed RN in Indiana without restrictions.
• 2+ years of clinical experience in case management or similar health care services.
• Intermediate to advanced computer skills (Word, Excel, Outlook).
• Excellent communication and interpersonal skills; ability to build rapport with diverse stakeholders.
• Ability to manage multiple priorities in a fast-paced environment.
• Live/reside in Indiana.

Responsibilities

• Coordinate all medically and non-medically required services for members.
• Develop longitudinal, trusting relationships to improve quality and continuity of care.
• Collaborate with Service, Transition, and other care team staff to meet member needs.
• Coordinate with Medicare payers, plans, and providers for eligible members.
• Serve as primary point of contact for the Interdisciplinary Care Team and external resources.
