The role requires travel up to 70% in Indiana and a valid Indiana RN license.

Hybrid work model with on-site member visits, 40-hour week, and a focus on coordinating with ICT and Medicare resources to ensure holistic member care.

Qualifications

• Licensed Registered Nurse (RN) in Indiana.
• At least 2 years of clinical nursing or case management experience.
• Proficient with Microsoft Word, Excel, and Outlook.
• Excellent communication and interpersonal skills.

Responsibilities

• Coordinate and manage member care across medical and non-medical services.
• Develop trusting relationships with members to improve quality and continuity of care.
• Collaborate with ICT and care team to meet member needs.
• Coordinate with Medicare payers, Advantage plans, and providers as appropriate.
• Act as primary contact for the Interdisciplinary Care Team (ICT).
