Qualifications

• Must hold a valid Indiana RN license.
• Minimum 2 years of clinical case management or related health care experience.

Responsibilities

• Develop longitudinal, trusting relationships with members to coordinate care.
• Coordinate with Medicare payers and providers to align benefits and services.
