Job summary

Humana Healthy Horizons in Indiana is seeking a Care Coordinator 2 to assess and evaluate members' needs and guide families toward resources that support wellness. This field-based role serves members of Indiana PathWays for Aging.

The position offers a hybrid work model with remote work at times and required travel of about 70% to meet with members. Indiana RN licensure and 2+ years of nursing and care coordination experience are needed, with strong communication, organizational, and computer

Qualifications

• Licensed Registered Nurse (RN) in Indiana with no restrictions.
• At least two years of clinical case management or similar health-care experience.
• Proficient in MS Word, Excel and Outlook.
• Excellent communication and interpersonal abilities.
• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Assess member needs and coordinate medical and non-medical services.
• Develop trusting relationships with members to improve care coordination.
• Collaborate with ICT and other care team members.
• Coordinate with Medicare payers and providers for eligible members.
• Serve as primary contact for the Interdisciplinary Care Team (ICT).
