Job summary
Healthcare Services Group, Inc. is seeking a Registered Dietitian for a hybrid schedule in a long-term care facility. You will assess residents’ nutritional needs, develop care plans, and document them in the EMR.
Collaboration with interdisciplinary teams will drive improved health outcomes. Responsibilities include counseling, education, and ensuring compliance with CMS/state/federal regulations. Travel across buildings may be required to meet facility needs; vaccination policies apply where
Qualifications
- RD credential required and current in-state licensure
- Strong organizational and communication skills required
- Compliance with COVID-19 vaccination policies as required by state and customers
Responsibilities
- Assess clinical nutrition needs of residents/patients and document plan in EMR
- Develop and evaluate individualized care plans based on findings, labs, and weight trends
- Provide nutrition education and counseling to residents/patients, families, and staff
- Collaborate with interdisciplinary teams to improve health outcomes
- Ensure adherence to CMS, state, and federal regulations and company standards
- Provide coverage across multiple buildings as needed
- All other duties as assigned
