Job summary

Community Connections, Inc. seeks a Case Manager to deliver intensive, community-based behavioral health case management for adults with serious mental illness and co-occurring disorders transitioning from incarceration to community living in DC.

Using a trauma-informed, recovery-oriented approach, you’ll help clients access housing, healthcare, benefits, and employment supports, coordinating services and maintaining care continuity during critical transitions.

Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Sociology, Criminal Justice, or related field.
• Minimum 2 years of experience providing behavioral health, case management, or social services to adults.
• Experience with serious mental illness and/or co-occurring substance use disorders.
• Experience working with justice-involved populations and community-based services.

Responsibilities

• Maintain an active caseload of approximately 20-25 participants.
• Provide person-centered, intensive support during pre-release planning and the first six months following community reentry.
• Adjust service intensity based on participant needs and risk factors.
• Coordinate behavioral health, substance use treatment, primary care, and social services appointments.
• Support linkage to Medicaid, housing, employment, and benefits programs.
