Job summary

Action Wellness in Philadelphia is seeking a LEAP Project Case Manager to support individuals affected by incarceration by linking clients to housing, healthcare, employment, and community services to promote successful reintegration. This role also involves collaboration with YesCare staff, the Defenders Association, court officers, and probation/parole offices to ensure seamless service delivery.

Qualifications

• Bachelor’s degree preferred in related field or equivalent experience.
• Minimum of two years’ experience with demonstrated growth in responsibility.
• Experience working with incarcerated individuals and reentry is preferred.

Responsibilities

• Conduct in-person needs assessments to identify client strengths and service needs.
• Develop and update individualized service care plans quarterly.
• Maintain accurate activity logs in accordance with program requirements.
• Provide case management and advocacy services for incarcerated/recently released individuals.
• Facilitate access to housing, healthcare, and community services.
• Collaborate with YesCare staff, Defenders Association, court officers, and probation/parole offices.
• Stay informed about available resources and provide referrals.
• Record client contacts promptly and complete intake documentation within 48 hours.
