Job summary

Colorado Department of Corrections seeks a dedicated case manager to support offender re-entry through advocacy-based, team-driven case management. You will conduct comprehensive assessments, develop plans, and coordinate access to education, treatment, work, and parole planning to promote successful community reintegration.

The role emphasizes data-driven planning, interdisciplinary collaboration, and ongoing progress monitoring within a corrections setting.

Qualifications

• Three (3) years of experience working directly with a hard-to-serve high-needs client population in a correctional or human services environment providing case management, including case planning, risk and needs assessment, and coordination of services.
• Must meet minimum and special qualifications for the vacancy and be able to communicate effectively in English.
• Experience with offender management processes and CTAP criteria is preferred.

Responsibilities

• Conduct initial assessment and admission data summary including criminal history, medical and social history, occupational experience, and program interests.
• Access CDOC information systems to retrieve and enter data from internal and external sources.
• Develop initial case management plans with offenders and coordinate access to education, treatment, employment, and related programs.
• Monitor progress, update case plans, and report findings to appropriate stakeholders.
• Maintain regular contact with offenders to coordinate care and ensure continuity from intake to release.
