Job summary

Arise Collective is seeking a Case Manager to support women reentering the community after incarceration. The role involves holistic assessment, resource referral, advocacy, and life skills coaching to develop personalized reentry plans.

You will work in a hybrid setting, coordinating housing, employment, healthcare, and education services, while maintaining case records and supporting program goals with a multidisciplinary team.

Qualifications

  • 5-8 years of progressively responsible experience providing direct services to clients.
  • Associate degree in social work, criminal justice or a related field; bachelor’s degree preferred.
  • Inclusive, empathetic worldview and demonstrated skill working across lines of difference.
  • Experience working with high-risk women and youth; trauma-informed modalities a plus.

Responsibilities

  • Develop and implement personalized reentry plans focusing on housing, employment, health care, education, and transportation.
  • Provide ongoing support, crisis intervention, and advocacy for participants.
  • Maintain confidential case records and coordinate with multidisciplinary teams.
  • Engage with community resources, employers, housing providers and service agencies to support reentry goals.
  • Participate in program development, evaluation and staff trainings.
