Catholic Charities of the Archdiocese of Washington is seeking a Case Manager to provide community-based, trauma-informed case management for individuals who are homeless or reentering after incarceration. You will liaise with DC agencies and local partners to identify resources and coordinate referrals.

Qualifications

• Bachelor’s degree in social work, human services, or related field.
• Licensure in social work in the relevant jurisdiction is required.
• 3 years of experience working with incarcerated adults, including those with mental disorders.
• Must pass a background check to access secured facilities.

Responsibilities

• Interview and assess direct service needs of Returning Citizens and others with mental health histories.
• Collaborate with DC agencies and community partners to coordinate services and referrals.
• Ensure clients attend group sessions on financial literacy, employment readiness, and related topics.
• Maintain complete case records and enter data within 48 hours of contact.
