Astrana Health is hiring a Population Health Coordinator-ACM in Orange, CA. This role supports member outreach, assessments, and documentation, with hybrid in-office and remote work options. The position requires interaction with ACM nursing staff and participation in audits and trainings to improve care delivery.

The candidate should have a high school diploma or GED and comfort with MS Office tools. Bilingual Mandarin/Cantonese is a plus, with a salary of $19.00-$22.00 per hour.

Formación

• High school diploma or GED is required.
• Experience using Microsoft Office applications (Word, Excel, Outlook).
• SNP Case Management experience is a plus.
• Mandarin or Cantonese language ability is preferred.
• MSO/IPA/Health Plan experience is beneficial.

Responsabilidades

• Outreach to members/providers by phone or written communication using department templates.
• Assist ACM nurses with member assessment, eligibility verification, and records requests.
• Attend health plan and ACM department meetings and trainings.
• Support ACM Coordinator I and team members and mentor as needed.
• Lead delegated projects and develop training for new coordinators.
• Perform other duties and projects as assigned.
