Job summary

MedZed is seeking a Community Health Navigator II in Pomona, CA. This field-based role manages an ECM caseload, conducts assessments, develops care plans, and enrolls members through visits and phone outreach. You will coordinate referrals and document in Salesforce Health Cloud, operating in a hybrid/remote setup with substantial travel.

The role reports to the ECM Territory Manager and requires 2+ years in ECM or community health, with strong compliance and documentation skills.

Qualifications

• 2+ years’ experience in ECM, case management, or community health.
• Strong documentation skills and ability to maintain compliance.
• Ability to handle complex psychosocial and medical needs.

Responsibilities

• Manage ECM caseload up to 50+ members with field visits.
• Conduct initial and follow-up assessments including PHQ-9 and other needed tools.
• Develop and update individualized Care Plans for members.
• Enroll new members via calls, visits, and door-knocks.
• Coordinate referrals to health plans and community resources.
• Document notes in Salesforce Health Cloud and health plan portals.
