Job summary

Family Practice & Counseling Services Network is seeking a Population Health Coordinator to support hybrid community health initiatives, grant programs, and care coordination across our network in Philadelphia. This role bridges frontline health work with program implementation under the Population Health Manager.

The position involves direct outreach, patient navigation, and collaboration with care teams to address social and logistical barriers to care, while ensuring program workflows align

Qualifications

• Associate degree or equivalent in community health, health administration, or related field required.
• Minimum 3 years of relevant experience in community health, care coordination, or program coordination.
• Experience coordinating multiple priorities and maintaining documentation across teams.
• Pennsylvania Certified Community Health Worker (CCHW) certification preferred or within 6 months of hire.

Responsibilities

• Coordinate day-to-day implementation of assigned Population Health grants and programs.
• Maintain program workplans, timelines, deliverables, and documentation.
• Translate grant requirements into practical workflows and tools.
• Coordinate activities across sites and support communication with internal teams and partners.
• Track progress toward deliverables and escalate barriers or delays.
• Prepare materials and data summaries for internal and funder reports.
• Maintain compliant program files and inventory records.
