MedZed is seeking a Bilingual Community Health Navigator II for a field-based role in Pomona, CA. This full-time position requires significant travel and offers a hybrid/remote work arrangement.

You will manage complex member cases, conduct in-field outreach, and coordinate care plans to reconnect members with primary care. The role emphasizes documentation accuracy, CalAIM/DHCS compliance, and ongoing member engagement, with opportunities to mentor newer staff and work across interdisciplinary

Qualifications

• Minimum 2+ years in ECM, case management, or community health.
• Proven ability to document accurately and maintain compliance.
• Willingness to conduct outreach via phone and in person.

Responsabilités

• Manage ECM caseload up to 50+ members.
• Perform initial and follow-up ECM assessments.
• Develop and update individualized Care Plans.
• Conduct calls, field visits, and door-knocks to enroll members.
• Coordinate referrals to health plans, providers, and community resources.
• Document notes in Salesforce Health Cloud and portals.
• Attend trainings and participate in team huddles.
